Review and improve, transfer of management and update of management agreements.

LD Floating Support Service.

Review and improve the LD floating support service.  The service had been left for a long time to tick over by both Selwood and Wiltshire Council.

There were no mechanisms in place to monitor, report or record outcomes and the
contract had not been reviewed for several years.

Outcomes

New processes and systems were implemented to enable recording and monitoring of service delivery and outcomes. The contract was reviewed with Wiltshire Council and a two year extension was agreed to give Wiltshire Council time to understand what the needs in the county were and whether the service remained relevant.

Transfer of Telecare Providers.

An agreement had been made to move the telecare management from Hanover to Tunstall and the role was to manage the transfer from the Selwood side.

This included co-ordinating the physical reprogramming of alarms with Selwood colleagues and the project team within Tunstall whilst maintaining a complete service for customers during the transition and monitoring progress to ensure tight end of project deadlines were met.

Outcomes

All alarms and systems were transferred prior to completion date whilst maintaining service delivery to customers. A report was delivered with analysis of the service costs v’s income including the sales and retention targets required to maintain the current income.

The report also highlighted the relevance of the technology in the future given the use of mobile phones and applications available.

Care Provisioning

Review, align and update care provider management agreements: There were multiple management agreements with six different social care providers.

The project involved creating a new management agreement and negotiating with all other providers to agree a number of changes to bring them all into one clear agreement with defined roles and responsibilities.

Outcomes

Five of the six organisations signed up to the new management agreement which included transferring responsibilities for specialist equipment to care providers and Selwood agreeing to complete void inspections.

In addition it was important to ensure all Teams understood the services being provided so a care home guide was created with a matrix of roles and responsibilities across finance, maintenance and housing management.
